Medicare revalidations past due, by state
The CMS revalidation file for October 2026 lists 227,096 organizations in the 51 states this site covers. 59,338 show a due date inside the last year that has already passed, and 16,274 of those are inside the 90-day clock — the window where a filing still lands before CMS may turn off billing privileges. Each state opens its own list of the twenty organizations whose date went by most recently.

Start with the number you have
One NPI is enough to answer the question this page ranks by state. You get the due date CMS published, the month of the file it came from, and how many organizations in that state are already past their date.

Which states carry the most past-due organizations
Ranked by how many organizations are inside the 90-day clock, because that is the count that changes by the week. The second column counts the wider problem: a date that passed inside the last year.

How to read these numbers
- Organizations, not enrollments. A group that holds four NPIs counts once. The CMS file itself has roughly ten enrollment rows per organization, so this table and the state reports under how it works will never add up — they count different things, on purpose.
- Past the date is not deactivated. CMS publishes a due date. It does not publish whether the 855 was filed, and a date often stays on the file after the paperwork went in.
- 90 days is the clock. 42 CFR 424.540(a)(3) gives 90 days from the due date before CMS may deactivate billing privileges. A date outside that window is usually a filing the file never caught up with, or an organization that stopped billing.
- Every count is checkable. Any name in any state list opens that NPI’s own page, and that page links to the row CMS published.
