Medicare revalidations past due in Pennsylvania
The CMS revalidation file for October 2026 lists 8,638 organizations in Pennsylvania. 2,768 of them show a due date inside the last year that has already passed, and 755 of those are inside the 90-day clock. These are the 20 whose date went by most recently — the ones a filing still rescues, and the ones somebody can act on today. A chain with hundreds of NPIs appears once, with the count beside it.

How to read this list
- Past the date is not deactivated. CMS publishes a due date. It does not publish whether the 855 was filed, and a date often stays on the file after the paperwork went in.
- One row per organization, not per enrollment. A group with three enrollments appears once, with the most recent date it holds inside the window. Individual enrollments are left off on purpose: that address is often a person’s home.
- The list starts with the dates that just went by. 42 CFR 424.540(a)(3) gives 90 days from the due date before CMS may deactivate billing privileges, so a date inside that window still has a filing that lands in time. That is why the newest dates come first and why the list stops at 90 days: a date older than that is usually a filing the file never caught up with, or an organization that stopped billing, and neither is something a phone call fixes.
- CMS issues due dates in batches. Several organizations share one date, so the top of this list is one date with the largest groups first. That is the file, not a sampling error.
- Every name is checkable. Each row opens that NPI’s own page, and that page links to the exact row CMS published.
